Triage Nurse

  • *This is a Monday - Friday 8am-5pm office based position.



    Using telephonic interaction as the mode of delivery to facilitate callers’ access to access caller’s needs. The triage nurse will utlizie Company protocols and policies to address after hours calls/needs and make assignments to on-call staff as indicated.


    • Must hold a valid and current nursing license in the state employed.
    • At least two (2) years’ experience in a hospice as a registered nurse.
    • Must be able to relate positively and favorably to patients and families and to work cooperatively with other employees.
    • Must have related administrative experience and education at a level necessary to accomplish this position.
    • Must have proficient computer skills
    • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
    • Must be capable of performing the job functions of this position with or without accommodations.


    • Must be able to rely on their communication skills and knowledge of disease processes to ascertain an accurate understanding of the patient’s symptoms.
    • Must have impeccable listening skills to notice the non-verbal clues the client is giving regarding pain, anxiety, fear and level of comprehension.
    • Ability to utilize judgment and decision-making skills in performing triage exemplifying sound clinical judgment.
    • Pleasant telephone manner.
    • Excellent customer service skills and personal service knowledge including assessment, meeting quality standards of service and evaluation of customer satisfaction.
    • Promote excellent clinical outcomes.
    • Documentation regarding all calls and outcome, including triage follow through, of each call.
    • Follow through the call tree to get a patient’s needs taken care of.
    • Triage nurse to follow up with agency, physician and any other entity as appropriate, with clear documentation of the disposition of patient on the call log, and matching documentation in the communication area of patient’s electronic record.
    • Maintain a working knowledge of federal and state regulation.
    • Develop positive relationships on behalf of the company in the program targeted for new market growth and the community at large.
    • Adhere to all of the Company’s policies and procedures.
    • Performs other duties as assigned.
